Just like any insurance that you buy, you have to pay a monthly premium. (If you can find someone that's going to give you insurance for free then lucky you!)
For dogs it will usually cost around $20-$50 per month depending on which company you go with and how much you want your plan to cover.
If you want it to cover routine things like dental and shots then it is going to make your monthly premium go up a lot. I have found that it is not really worth it to include routine care in the insurance plan but rather pay for it yourself.

Personally, I recommend Trupanion because it only costs about $25 a month and it has no annual limits so if your pet has a lot of accidents in one year you don't have to worry about 'max'ing out your insurance for that year.

It depends on the health care clinic you go to. Usually, yes.

I'm pretty sure you do- I think the idea for the company is that the premium makes them more money in the long run- taking the risk that your dog won't have that many health problems (or at least, problems that are less expensive than what you pay in premiums.)

On the other hand it saves you from having to pay for things like surgery all at once so...

I'd say it depends on your insurance company.
